East Suffolk Primary Care Network (ESPCN) is seeking an experienced Cancer Care Nurse. The post will cover mid-Suffolk (Eye to Stowmarket), Ipswich & Felixstowe for 30 hours per week.

ESPCN is a collaborative network of GP practice in East Suffolk. We work together along with community, mental health, social care, pharmacy, hospital, and voluntary services to provide integrated health and care services to our 130,000 patients. The role will be employed by Suffolk Primary Care, an at-scale primary care provider covering 8 practices in East and West Suffolk.

Main duties of the job

Your Role

As a PCN Cancer Care Nurse, you’ll play a pivotal role in driving forward our cancer care agenda. Your responsibilities will include:

  • Conducting Cancer Care Reviews for our patients, ensuring the very best personalised care and support.
  • Supporting 2WW suspected cancer pathway referrals into secondary care, ensuring all pre-referral tests are completed.
  • Scoping community services to aid in early cancer identification and linking into local communities and voluntary sector organisations.
  • Leading the development of health promotion pathways and driving improvements in the quality of Cancer Care Reviews in primary care post-diagnosis.
