Care Navigator – Derby Road Practice
As part of Suffolk Primary Care, which is a group of 7 GP practices across Suffolk, we are seeking to recruit an enthusiastic and flexible Care Navigator, with strong customer service skills, for Derby Road in Ipswich who look after over 18,700 patients.
Derby Road have 2 sites, based in the heart of Ipswich, with close travel links to the beautiful Suffolk countryside and the Suffolk coast.
We have an established team with a strong focus on customer service and communication. We would love to hear from candidates with GP surgery experience although we would also like to hear from those that can show flexibility and a range of transferable skills.
The successful candidate should be able to work calmly and professionally as part of a team. You should have the ability to multi-task. A strong command of written and spoken English with an excellent telephone manner is a must. Good time management skills and the ability to follow protocols and or instruction and attention to detail is essential.
Over the last few years, we have developed our unique GP-PA hub a team of highly skilled care navigators and duty doctors working together to triage all incoming requests. We use the Ask My GP online consultation service allowing us to manage our own workload with control over face-to-face appointment lengths and timings. We have a large multi-skilled multidisciplinary team of ANPs, a paramedic, pharmacists and pharmacy tech to support the wider clinical team.

Main duties of the job
- To provide comprehensive administrative support for the general practice and to act as focal point for patients and staff.
- To receive and assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
- Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors either in person or via the telephone.
- Facilitate effective communication between patients and members of the healthcare community.
- Handle patient complaints in accordance with practice protocols.
- To provide an efficient and accurate support to the secretarial team, clinicians & patients.
