Job summary
We are looking to recruit a Pharmacist to join our Pharmacy Team, to develop and deliver medicines optimisation services across our East Suffolk Primary Care Network (PCN). East Suffolk PCN is made up of 12 practices and more than 120,000 patients. The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. We offer flexible roles dependent on the hours you are available for work and your level of experience.
The successful applicant will work as part of a multidisciplinary team to develop and deliver the PCN DES (Direct Enhanced Service) and run practice clinics. In return, we will support you to develop, working in an environment that nurtures and develops quality. We are looking for independent prescribers or those willing to or working toward the qualification. Experience in General Practice would be an advantage but is not essential if you are clinically up-to-date and understand the process of full a medication review within a Primary Care setting.

Main duties of the job
The key responsibilities for the pharmacist are:
- To reduce the medicines related workload on doctors
- Increase safety and quality of prescribed medicines
- Provide support on medicines related issues to the whole practice team
- Be a point of contact for patients regarding medicines queries
