Join Our Team as a Senior Pharmacist Supporting General Practice

We are searching for a highly skilled Senior Pharmacist to join our team and play a pivotal role in supporting general practice. If you have a wealth of pharmacy experience and are eager to make a significant impact, we want to hear from you!Your expertise and leadership will be instrumental in optimizing medication management and enhancing patient safety. You will collaborate with a multidisciplinary team to ensure the highest standard of pharmaceutical care and provide guidance on complex medication-related issues.
The successful applicant will work as part of a multidisciplinary team to develop and deliver the PCN DES (Direct Enhanced Service) and run practice clinics. East Suffolk PCN is made up of 12 practices and more than 120,000 patients.
Main duties of the job
Undertake clinical medication reviews with patients with multimorbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests
Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review)
Medicines reconciliation on transfer between care providers
Care home/Frailty medication reviews
Managing caseload of patients with common/minor/selflimiting ailments while working within scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate
