NHS Health Checks – Provider Resources & Information
Welcome to Suffolk NHS Health check Resources
Background: Suffolk GP Federation (The Fed) is offering the NHS Health Check Programme in Suffolk & Waveney. The NHS Health Check is a prevention programme which aims to reduce the chance of a heart attack, stroke or developing some forms of dementia in people aged 40-74.
It achieves this by assessing the top seven risk factors driving the burden of non-communicable disease in England, and by providing individuals with behavioural support and, where appropriate, pharmacological treatment.
The NHS Health Check is not suitable for individuals who already have cardiovascular or other related diseases, such as Coronary Heart Disease (CHD), Chronic Kidney Disease (CKD), Stroke, and Diabetes Mellitus (DM).
