Suffolk GP Federation manages various community services in alliance with East Suffolk and North Essex NHS Foundation Trust, West Suffolk NHS Foundation Trust, Suffolk County Council and Norfolk and Suffolk NHS Foundation Trust.

Stoma
East Suffolk Community Stoma Care provides support for patients with a stoma, working in partnership with acute trusts to create a seamless pathway of care.
We enable patients and relatives to develop skills to self-manage their long-term condition. Patients are seen initially at home or in intermediate care and then, if able, reviewed in outreach clinics. The service sees patients with a colostomy, urostomy, ileostomy, fistula or wound that require a drainage bag.

Monday to Friday: Opening times vary depending on location. Morning and afternoon clinics are available.

Referrals come from the following sources:

- Other stoma care nurses
- Lost Ostomist
- Multidisciplinary teams
- Acute hospitals
- Care coordination centre
