Care Coordinator – West Ipswich Care Management Service (Internal Candidates Only)

We are excited to share an opportunity to join the West Ipswich Care Management initiative which supports high users and predicted high users of acute non-elective beds. Care Management is a way of intensively supporting this group of patients with proactive care co-ordination and additional GP & Community Matron capacity.

We have started this service in the West Ipswich Integrated Neighbourhood Team area and our aims are:

• To improve the care of these complex patients
• Ease workload pressures across the system
• Reduce hospital admissions

We are now seeking a Care Coordinator to work as part of this multi-disciplinary team. You’ll support the co-ordination of support to individuals and families across multi-disciplinary services within the Care Management Service. This involves processing referrals and ensuring patients get the right support from the multidisciplinary service at the right time. You’ll be the central point of information and guide processes to improve working practices.

We are looking for individuals who thrive when working across organisational boundaries to deliver the best care for this group of patients, who have excellent organisational skills and can build strong and lasting working relationships quickly.

Contract details

• Permanent
• Working days: Thursdays and Fridays, up to 15 hours per week
• Location: Constantine House, with ability to travel to West Ipswich Practice sites
• Salary: £27,485 – £30,162 WTE
